After an agent has been through the training process, he/she should submit the required licensing documentation to the Broker Operations department via his/her appropriate regional Account Executive. The required licensing documentation will include the following: EFT Authorization Form, Appointment Instructions, Certificate of Completion, Application for Appointment, W-9, Producer Agreement & Addendum.
